By clicking the link above you will be directed to the PWCS Office of Student Services website which will provide you the standard registration forms for Prince William County Public Schools so that you can print and complete the forms in advance of registration if you choose. In addition, Forest Park High School may also have additional forms that are specific to the individual school. Students wishing to participate in activities at Forest Park High School need to be registered & enrolled at Forest Park High School prior to attending any in season or out of season events.

VHSL Physical Form 2015-2016 School Year
All physical examinations must take place after May 1, 2015 for a student-athlete to participate in sports at Forest Park High School for the 2015-2016 school year. The examination results, all information, and physician's signature must be on this form and submitted to the activities office prior to tryouts as well as out of season conditioning. You can access a copy by clicking the blue link above.

Emergency Permission Form
Once completed, print emergency permission form, have parent/guardian sign the bottom of the form, then turn in hard copy to Ms Shanks (FPHS certified athletic trainer) - you may turn this in to the activities office to be put in her mailbox. NO electronic submissions will be accepted.

Face to Face Concussion Training Presentations
All high schools are required to provide Concussion Presentations for parents and students according to PWCS Policy 759. As there are sufficient differences in the way concussions will be handled at the middle school and high school levels, a parent having children in both middle and high schools are required to attend both a middle and a high school presentation.

On-Line Concussion Training. Virginia Code 22.1-271.5 requires all student-athletes and their parent/guardian to complete a concussion education program every year. The PWCS On-Line Concussion Training course is designed to be compliant with Virginia Code 22.1-271.5. Parents and students who have already attended a face-to-face training may take this year’s mandatory concussion training online. It is to be completed with parent/guardian and student TOGETHER. Once complete, print completion certificate, have parent/guardian sign, then turn in hard copy to Ms Shanks (FPHS certified athletic trainer) - you may turn this in to the activities office to be put in her mailbox. NO electronic submissions will be accepted.

PLEASE do not wait until the last minute (the day/night before try-outs) to complete the training, as it will overload the system and you will have trouble completing the training.  You can access the online concussion training by clicking the blue link above.


Transfer Regulation Regarding Eligibility 
VIRGINIA HIGH SCHOOL LEAGUE ELIGIBILITY INFORMATION ADOPTION OF PWCS REGULATION 721-2
STUDENT TRANSFERS - HIGH SCHOOL ATHLETIC ELIGIBILITY


With the adoption of the new PWCS Regulation 721-2, changes have been added that impact a student’s Virginia High School League eligibility for participation in interscholastic sports at the high school level. Briefly stated, the following excerpts from Regulation 721-2 describe possible impact if a student transfers from one school to another:
  • Once a student establishes eligibility in a high school, any additional transfer requests for designated site programs shall not be considered for a waiver and the student shall be ineligible for 365 days from the date of the transfer.
  • Students who enter the ninth grade through residence or acceptance into a designated site program will establish Virginia High School League (VHSL) eligibility at the school.
  • Once a student establishes eligibility at a high school and the student transfers to another high school for any reason, the student shall be ineligible for 365 days unless there is a corresponding change of address in accordance with the VHSL
    Regulation 28A-7-1.
